Apparently the CentOS builds of anki-sync-server don't depend on X, but anki-sync-server does indeed depend on anki. For this to be -headless, either anki needs to be changed (to either have a -headless variant, or to only "optionally" depend on all the stuff which pulls in X), or anki-sync-server needs to "provide" its own anki binaries.

Is "anki" strictly speaking necessary as a dependency here? Nothing I see in the PKGBUILD or the upstream README seems to mention anything about GUIs or X, but the presence of "anki" in the dependency list would compel my headless server to pull a whole ton of totally unnecessary packages.

What did you change it to? 700?

Had to change permissions for /var/lib/anki-sync-server/collections. Can this be implemented in PKGBUILD?
