How to enable this theme:

I Change the theme to "Fallout" in GRUB

Set GRUB_THEME="/usr/share/grub/themes/fallout/theme.txt" in the file /etc/default/grub

Make sure the theme file is indeed valid!

II Update GRUB

# grub-mkconfig -o /boot/grub/grub.cfg
Generating grub configuration file ...
Found theme: /usr/share/grub/themes/fallout/theme.txt

@rzl The PKGBUILD appears to be installing all the PNGs in the source repo's icons folder into /usr/share/grub/themes/fallout rather than /usr/share/grub/themes/fallout/icons as is needed for the icons to work. Can you please fix this?

This theme is great! For others, a GFXMODE of auto in /etc/default/grub results in grub guessing a resolution that is much too low. My laptop is 1280x800 and that looks fine.

tbh i don't really know if there's some guidelines about this in particular but i just thought that naming the dest folder fallout-grub-theme when it's already in grub/themes felt kinda redundant, as my others already installed theme are just named simply breezy or starfield

You're right. Frankly, I just followed the upstream convention, but I don't feel strongly either way. Is there a guideline on this?

Why installing the theme in /usr/share/grub/themes/fallout-grub-theme instead of just /usr/share/grub/themes/fallout? it's kinda redundant.
