The system-monitor-applet-config.desktop says "Exec=system-monitor-applet-config.py", but in the PKGBUILD you install system-monitor-applet-config.py as /usr/bin/system-monitor-applet-config.

Glass errors tab? Refresh times are all 1500. Also, this seems to happen nearly immediately after I run the config gui and exit it

There are several logs which can help: in looking glass errors tab and by launching gnome-shell --replace in a terminal. What are your values of refresh time ?

Looks like the extension is "frozen" on my system, that is it does not update (cpu/net/disk). All graphs are frozen. Is there a log I can send you to debug?
