In short, vbox use its own install/removal mechanism for extensions.

Any good reason why this package just owns a binary blob and copies some files outside package management while installing?

virtualbox-ext-oracle.install directs pacman to run the following on install:
VBoxManage extpack install "/usr/share/virtualbox/extensions/Oracle_VM_VirtualBox_Extension_Pack-${1%%-*}.vbox-extpack" >/dev/null
In doing so, modifications outside of the package itself are done to the filesystem. Is there a way to have VBoxManage redirect the output of that command to the $pkgdir/right/path thus keeping these files owned by the package?

When reinstall, it remove old extpack before install again. So I don't see why you have an issue.

In the install file, please modify the post_upgrade() adding the '--replace' flag to VBoxManage extpack install, otherwise it might failt to install the extension in VBox.
Message:
"VBoxManage: error: Extension pack 'Oracle VM VirtualBox Extension Pack' is already installed. In case of a reinstallation, please uninstall it first"
Steps to reproduce:
- installed this package
- reinstalled (as it didn't show up before in VBox) - removed '> /dev/null' to get error message.

There is no version dependency on this package on purpose! You could read comments back from 2011 to understand why.
